About the Function:
As a POH somatics, you will work temporarily and independently on the care for patients with chronic conditions such as diabetes, asthma/COPD, and cardiovascular diseases. You perform medical checks, guide patients through lifestyle changes, and give advice on medication use. In addition, you run consultation hours, during which you conduct intake interviews, take anamneses, and perform medical procedures. You work closely with general practitioners and other healthcare providers within our multidisciplinary team. In doing so, you make use of digital tools such as Medicom to organize care efficiently and patient-oriented. You will be given the space to think along about improvements in care processes and share knowledge with colleagues. Together we ensure accessible, high-quality care in a modern practice environment.

Job Requirements:
We are looking for an independent and experienced POH-Somatics with an affinity for innovation and patient-oriented care.
- Registered nurse with Post-HBO Practice Nurse General Practice (Somatics) diploma.
- Minimum of 1 year experience in primary care as a POH-S.
- Knowledge of and experience with Medicom and CVRM.
- Experience with spirometry and COPD is a plus.
- Available on Mondays and flexible in cooperation.
